Normalize formatting/line endings across configs, controllers, models, tests, and OpenAPI specs. Update rule expression/rule engine implementation and remove obsolete RuleAction controller/model. Add unit tests for rule expression syntax and multi-action behavior, and include docs updates.

# Suggested Commands (Windows)

## Core Project Commands
|
|
- Install dependencies: `composer install`
|
|
- Run all tests: `./vendor/bin/phpunit`
|
|
- Run one test file: `./vendor/bin/phpunit tests/feature/Patients/PatientCreateTest.php`
|
|
- Run one test method: `./vendor/bin/phpunit --filter testCreatePatientSuccess tests/feature/Patients/PatientCreateTest.php`
|
