fix(api-docs): update OpenAPI spec to match actual implementation

- Update Occupation schema (OccCode, OccText)
- Update MedicalSpecialty schema (SpecialtyText)
- Update Counter schema (CounterDesc, CounterValue)

- Update PatVisitADT doctor fields to integer (ContactID refs)
- Add proper request/response schemas for all endpoints
- Fix OrderStatus and Priority enums
